Any student wishing to participate in a sport will need to have a sports physical (PPE) completed and turned into the nurse's office. The physical must be within 365 days of the first practice of the season. If the physical is more than 90 days old, a Health History Update must be completed by a parent or guardian. 

Physicals may be mailed to the school, faxed, or dropped to the main office over the summer. Please be sure they are labeled "Attn School Nurse". These physicals need to be approved by our school physician prior to any student participating in fall sports. Parents will be notified if their student's physical was not approved.  

Please read the attached FAQ information sheet regarding sports physicals to ensure proper completion. Incomplete physicals will be returned.

You will also need to complete the "Athletic Release Forms for 2023-24" which include the Cardiac, Concussion, and Opioid forms. All are located in the Genesis Parent Portal on the Student Data page under the Athletics tab
